Any personal information entered in the following form is used to receive incident reports and address them, and reply to party submitting the information.

In some cases we will provide personal information we have obtained, such as a customer's name and contact information, to one or more of our affiliates, by paper or electronic medium, when we have determined that it is appropriate to respond to a product inquiry. In such cases, customers are able to request that the Company stop providing their personal information to affiliates.

We do not obtain personal information using means such as cookies or Web beacons. Those methods do not allow an easy identification and we prefer to explicitly request any required information.
